All Students must meet the minimum Texas immunization requirements prior to school entry to public school. Please refer to the Texas Department of State Health Services web page for the most current information, http://www.dshs.state.tx.us/immunize/default.shtm.
To obtain, and print, your child’s current school shot record:
Go to: Family Access
Under General Information, choose Health Information, and print the shot record.
** If your student receives any immunizations during a healthcare provider visit, please bring in the updated record, to your child’s current school nurse. We will enter the new immunizations to your child’s school record. **
7th grade Immunization Records – Summer 2017
1 dose of Tetanus/Diphtheria/Acellular Pertussis (Tdap), if at least 5 years have passed since the last dose of tetanus-diphthereia (TD or Tdp), and 1 dose of Meningococcal conjugate – vaccine must be identified: Menactra, Menveo, MenACWY or MCV4
Students must have updated, current immunizations on file in order to attend classes the first day of school, August 21, 2017.
If your student has been given the required immunization(s), please get the paperwork to your current school nurse, prior to, Thursday, June 1, 2017. It will make the transition to Ereckson Middle School easier.
Between the dates of June 3--August 9, 2017, parents of incoming 7th grade students: You may Scan and EMAIL your student’s updated immunization record to your child’s middle school nurse, anytime during the summer:
Send a copy of your student's immunization record, with your student on, Day at the Pound.
Do not fax, or have your healthcare provider fax, your student’s updated immunization records to the school between the dates of June 3-August 9, 2017. The fax may not be received.
Student’s Name, Date of Birth, and healthcare provider’s signature and/or stamp, should be on ALL student immunization records.
Parents are encouraged to bring a copy of your child’s updated immunization record the evening of 7th grade schedule pick up, if you have not verified your child's school immunization status in Family Access.For immunization records which are sent via email to me during the summer: I will not be working during the summer, however, immunization records may be updated in Family Access periodically. Once the updated immunization information has been entered, parents may verify student’s health record in Family Access.
