• Two Proofs of Residency to include your lease, house agreement, or current voter's registration card and one current utility bill (gas, water, or electric.)
• Student's immunization records.
• Birth certificate or passport.
• Social security card.
• Previous School Withdrawal form (This is required during the school year, but not for Fall Semester enrollments).
• Middle and/or High School transcript and/or report cards.
• Parent or Guardian must provide a valid driver license or ID.
• Parent or Guardian must be present with student to enroll.

You can pick-up an Enrollment (forms) packet at Lowery Freshman Center or you can click on and print the enrollment forms below. Once you have scheduled an appointment, please bring the completed forms and the items listed above to the enrollment office for a faster enrollment process.
